DETAILS AND OVERVIEW OF PRINCIPLE APPROACH

These cases are advanced dental treatment. They follow the complete program of reconstruction techniques made on a fully adjustable articulator with the appropriate records and can be made in a variety of materials or mixed materials where requested.

Often these cases are long-term treatments if there are TMJ issues or other symptoms that requires additional treatment. Most cases are involving TMJ pain and most of these cases are often referrals to specialized dentists or Prosthodontists. Planning and an appointment schedules are often required.

Our objective is simplification as we are now in a very different time than when the science of Gnathology began in 1921 by Dr.B McCollum, Charles E.Stuart and others that followed at that time with the pantograph developments and the Stuart articulator. No one wants to go back to those days simply because it was immensely time consuming. Now is the time to get the same results, but without the time frames that existed and with far better aesthetic outcomes. It was often the case these cases spanned eighteen months to completion. Is iy now possible to reduce the times frames to modern acceptanble four or five stage appointments. The answer is yes.

BASIC PROCEDURE;

A cement retained case involving twenty eight teeth is far easier than an implant case as the techniques are well defined in classical reconstructions. The limitations are the position of the natural dentition and how much corrective adjustment can be made in temporaries, (treatment crowns), to attain a result that can be reproduced accurately into final ceramic restorations. This is a critical part of the case, so we use digital waxing because it can be adjusted and re-produced, or re-scanned as required. Each stage is then finalized on a fully adjustable articulator (STUART-53 or an OSS-OPTOM-(C)

The casework involves simple to follow techniques with pre-planning of stages which includes DE PROGRAMMING THE PATIENT to achieve correct mandibular alignment to CR. Here dentist and technician can collaborate to refine treatment options. That then is started with either scans or impressions and also requires specific photography and must include a basic bite reg and a basic protrusive bite, plus a face bow recording to attain the occlusal inclination. This is often the starting point that will include equilibration of the standing dentition in the pre-stages to determine the primary vertical dimension and primary occlusal configuration. Once in the laboratory we can create the first set of primary treatment crowns in HIPC composite, not PMMA. We then use this stage to de-program the mandibular position with a removable de-programming removable overlay. Once this stage is complete we can then take a final bite registration and then move to stage two.

When a restorative procedure such as this are performed then the aesthetic component can be completely restored as well. This science is the best approach to a complete reconstruction for patient with dentate dentition or the edentulous patient who will undergo complete implant rehabilitation.

Our direction now is to use our knowledge and skills to simplify the original science of Gnathology to being a more practical and less time consuming approach to the five principles of occlusion to develop more accurate outcomes. Techniques that integrates both analogue and digital technology to condense and assist the technical stages required. New techniques and instruments are under development also so protocols will fit into 2025 more effectively with the use of digital assist.
